The Tendon Tug-of-War
A hammertoe isn’t bone growth; it’s the result of muscle and tendon imbalance.
- Two distinct groups of tendons control your toes. One group pulls the toe straight, and the other group bends it downward to grip the ground.
- When you have an underlying biomechanical issue, such as severe flat feet or abnormally high arches, these tendons must work overtime to keep you balanced.
- Eventually, the tendons that pull the toe downward overpower the tendons trying to keep it straight.
This constant, uneven tension forces the middle joint to buckle and pop upward, creating that distinct claw-like appearance.
The Flexible Phase: Your Window of Opportunity
When a hammertoe first develops, it enters what podiatrists call the flexible phase.
- During this time, the joint is bent, but you can still use your fingers to physically press the toe flat against the floor.

- Because the joint is trapped in a bent position for months or years, the soft tissues shrink, and the joint calcifies and freezes in place.
- At this stage, you can no longer press the toe flat. It is permanently locked into a bent angle.
- Once a hammertoe becomes completely rigid, conservative treatments like taping, stretching, and padding will no longer work.
Surgery becomes the only medical option available to physically cut the tight tendons, remove a small piece of the frozen bone, and realign the toe.
